HOW TO REQUEST YOUR NC ELIGIBILITY CERTIFICATE FOR THE DMV

1. Student finishes the Driving part of Driver's Ed.

2. The instructor fills out the Certificate completely, including signatures.

3. The Driver's Ed teacher or student turns in the form to Ms. Amy Hill in Student Services.

4. Once the student has an appointment at the DMV, the student/parent will email Ms. Amy Hill at amy.hill@cabarrus.k12.nc.us requesting the Eligibility form. In the email the following must be included:
a. Student's Full Name
b. Date of Birth
c. PowerSchool #
d. Date of the DMV appointment or walk-in date

5. The Eligibility Form will then be filled out and an email will be sent back to the parent/student informing them that the certificate is ready for pick up in our front office.

6. Eligibility forms will only be completed for those in which a Driver's Ed Certificate has been turned in and who are academically eligible (passed 3 out of their 4 classes from the previous semester).
